I, being the parent/legal guardian of said minor, consent this minor to participate in the activity of Wednesday Night Youth Group including the following event within the activity: Walking trips to local locations, Pool Parties at Dave Yeggy's home, and Bus rides to & from Church. I recognize there are risks involved in participating in this activity and hereby assume all risk of injury, harm, damage, or death to my minor child associated with this activity and exonerate this church and all participating staff from any liability whatsoever resulting from these risks caused by said minor. If medical help is necessary, efforts will be made to contact me prior to treatment. If contact cannot be made, I authorize this church to contact medical help including 911 for necessary medical treatment or emergency health actions of said minor when necessary. I will not hold this church financially responsible for the emergency care of said minor. I agree that my insurance policy is the primary coverage and any insurance policy of the church is the secondary coverage.
